We use this consent form for projects which we deliver with members of the public. This is a simple/general consent form – some projects require more information from participants and you will need to ensure you capture what you need for your specific project.

It can be adapted to meet local context – please make sure you always fulfil your legal/ethical duties when it comes to looking after participants. Feel free to amend or adapt.
